Setup will not complete — terminal does not open

Installed the workflow. When invoked I do get the "Sign in and update items ..." banner but the terminal app doesn't open as promised. Any ideas?

As @Acidham pointed out, when asking about an existing Workflow it’s best to use its own forum thread. You’re probably asking about the 1Password one. See the second post, you probably have custom code in Alfred Preferences → Features → Terminal.
