Thanks @vitor, i've figured out my issue. op version was behind, but that wasn't the issue, although the issue was with op & authentication. Unfortunately i wasn't able to see much in the workflow logs. I have 2 accounts, personal & work, both of them enabled. Disabling work account and forcing update started to work after a while. So I tried listing work account vaults via op. That's when I got prompted for MFA. After authenticating and listing vaults via op, I re-enabled the account in the workflow and things seem to be working now. Some tips (in terminal): Try listing vaults via op: "op vault list". Check for presence of 2nd account vaults. If 2nd account vaults aren't present, try force listing vaults from that account: "op account list" (copy the user id). Then execute "op vault list --account <user id goes here>". This is when you'd get asked for MFA. That's it, now things seem to work. I can't imagine op or 1Password behavior changed; I imagine @vitor, you may have changed the way the some outputs are handled, which triggered the issue. The authentication session to my 2nd account may have been expired for months if not longer - i'll have to keep an eye on this. It's a business account that uses Duo for MFA. Also just noticed that process op is at high CPU utilization. If killed, it restarts, asking for TouchID authentication.

I tried the extras - accounts are showing up as-expected but vaults aren't. Force Update & Workflow reset didn't do much. I even deleted the workflow and reinstalled it. Machine was also rebooted.
